-
OFFICE HOURS: Our office is generally open 8:00 am to 6:00 pm Monday through Friday. Catherine Tran, our Intake, Billing, and Office Manager works remotely and is available by phone and email. In-person meetings may be arranged by request. The office number is 756-0555. Catherine's voicemail extension is 101. You may leave a message for anyone in the group at their voice mailbox. If you call after 5:00 PM during the week, or if you call on the weekend, you may leave a voice mail message or, in an emergency, follow the voice mail instructions to page our on-call therapist. If you are experiencing a mental health crisis and are worried about your safety, please call 911.
-
APPOINTMENTS: Appointments are made on a regular weekly or bi-weekly basis. When you schedule an appointment with your therapist that hour is specifically reserved for you. In the event that you are unable to keep your appointment, you must cancel or reschedule at least 24 hours in advance. If you fail to cancel or reschedule with 24 hours notice you will be charged your regular rate for that time. Your therapist will discuss this and other client-therapist policies with you in the first session.
-
PAYMENT AND INSURANCE COVERAGE: Payment for professional services is expected at the time the services are delivered. If this presents a financial hardship for you please discuss this with your therapist during the first hour. If you expect medical insurance to cover all or part of your counseling fees, there are a few things you need to know. First, each insurance policy has different rules and benefits. It is your responsibility to read your insurance booklet or contact your insurance representative to learn the specific requirements and coverage for your plan. In some cases, a referral from your physician is required, and/or sessions may need to be authorized by the insurance company before you begin counseling. This is almost always true with an HMO or managed care plan. Please remember that your medical insurance is a contract between you and your insurance company. It is up to you to make sure you have complied with all requirements of that contract in order to receive insurance benefits.
Payment of your account is your responsibility whether or not your insurance provides coverage. As a professional courtesy to our clients, we will submit insurance claims on your behalf. To do so you must furnish us with all the necessary insurance information including policy number, group number, your ID number, and name and address of your insurance company. Please feel free to ask your therapist or Catherine if you have any additional questions.
Again we would like to welcome you to our practice.
